What we’ll cover

At ME Physio, we have a range of unique services designed to assist with your independence and stay healthier, happier and stronger. This includes our Support ME programs which include Strength and Balance programs, Hydrotherapy and Clinical Strength and Conditioning. All of these services can be funded by your Home Care Package or Commonwealth Home Support Program.

These programs are tailored specifically for your needs to maintain and improve your independence. As part of our Support ME programs, your physiotherapist will complete a thorough assessment of your mobility, flexibility, strength and balance. These assessments allow our physiotherapists to tailor a program specifically designed to improve your mobility and maintain your independence.

All our Support ME Programs are conducted by our experienced physiotherapists and are eligible under the Home Care Package (HCP) and Commonwealth Home Support Programs (CHSP). These packages and programs can be utilised to assist in the funding of your physiotherapy at ME Physio.

Physiotherapy on your Home care packages

What is the the difference between Home Care Packages (HCP) and Commonwealth Home Support Program (CHSP)?

Both programs are government-funded home care services and applications are through the My Aged Care designed to assist older adults continue staying at home.

The main differences between HCP and CHSP is the amount of support required and the number of services to assist in maintaining your independence. These services include things like allied health services, nursing, respite or domestic services such as personal carers or cleaners.

CHSP is intended for older adults who may require one or two basic services. HCP are for older adults who need more services and there are different types of packages ranging from basic to higher needs.

How do I get started with physiotherapy under a Home Care Package?

To use your HCP at ME Physio, let our friendly Client Experience Team know when booking your initial appointment. In cases where your Case Manager contacts our clinic, the team at ME Physio will liaise and work directly with your case manager.

Following the first 1 – 2 appointments, our physiotherapists will create a plan of your health goals that we agree are most important to work on together. We then share the agreed plan with you, your family and your HCP manager. This plan allows everyone to collaborate to assist in keeping you healthier and stronger. Your treatment plan will include what we are working on, for how long and the costs that are involved.

Can Physiotherapy improve my mobility and balance?

Physiotherapy has been shown to greatly improve your mobility and balance. Combining specific exercises and recommendations for you has been shown to improve your strength and balance which will improve your overall independence. Our Support ME Programs improve and maintain strength and reduce the likelihood of falls. These programs can be done one on one with your physiotherapist or in a group class. Our group classes provide a fun and social environment in a specific physio gym.

Regardless of whether you have exercised before, it is never too late to start to improve your health and fitness.

Are these CHSP and HCP physiotherapy services available at home?

Our physiotherapists at ME Physio are able to provide both in-clinic and in-home services. We are currently able to offer Home Visit services under the HCP and CHSP to the following suburbs: Malvern East, Malvern, Glen Iris, Ashburton, Caulfield and Carnegie.
