At ME Physio, we offer physiotherapy, hydrotherapy and a range of exercise services under the Commonwealth Home Support Program (CHSP) and Home Care Packages (HCP). These services can be funded and/or subsidised if you have an approved CHSP or HCP and require physiotherapy to assist you managing conditions to assist in maintaining your independence.

At ME Physio, we have a range of unique services designed to assist with your independence and stay healthier, happier and stronger. In additional to physiotherapy treatment, this includes our Support ME programs which include Strength and Balance programs, Hydrotherapy and Clinical Strength and Conditioning.

These programs are tailored for your needs to maintain and improve your independence. As part of our Support ME programs, your physiotherapist will complete a thorough assessment of your mobility, flexibility, strength and balance. These assessments allow our physiotherapists to tailor a program specifically designed to improve your mobility and maintain your independence.

What conditions can be managed under My Aged Care packages?

CHSP and HCP physiotherapy can benefit people to help manage their condition and live independently and actively. This may include:

  • Conditions such as arthritis, osteoporosis
  • Chronic conditions such as heart disease and cancer
  • Neurological conditions including stroke and Parksonson’s disease
  • Alzheimer and Dementia
  • Falls assessment and balance re training.
  • Acute conditions such as back pain or joint pain.

Physiotherapy under the CHSP and HCP can be for ongoing conditions or short-term management of acute conditions that may affect your independence and wellbeing.

How do I get started with physiotherapy under a My Aged Care Package?

At ME Physio we work closely and accommodate a range of providers and case managers under CHSP and HCP. When you book your initial appointment, let our Client Experience Team know if you have an approved HCP or CHSP from My Aged Care. In cases where your case manager contact our team, we will reach out directly to yourself. Our team will liaise directly with you and your case manager to provide services to assist in maintaining your independence and activity.

Following your initial appointment, your physiotherapists will create a plan of your goals that you wish to address and work on together. We then share your plan with your CHSP/HCP case manager. This plan allows everyone to collaborate and assist in keeping you healthier and stronger. Your treatment plan will outline the type of physiotherapy services required, duration and frequency of treatment and costings.

Are CHSP and HCP physiotherapy services available at home?

Our physiotherapists at ME Physio are able to provide both in-clinic and in-home services. We are currently able to offer Home Visit services under the HCP and CHSP to the following suburbs: Malvern East, Malvern, Glen Iris, Ashburton, Caulfield and Carnegie.

What is Home Care Package physiotherapy? 

Home Care Package physiotherapy provides physiotherapy assessment and treatment that can be fully funded or subsidised by your Home Care Package or Commonwealth Home Support Package.

These services can include a range of physiotherapy treatments and services at ME Physio including physiotherapy, hydrotherapy, pilates and other exercise programs.

Who is eligible for Home Care Package physiotherapy? 

If you have a Commonwealth Home Supported Package (CHSP) or Home Care Package (HCP), you are eligible for physiotherapy treatment to manage or treat conditions that will assist you maintain your independence.

This can include the management and treatment of conditions including:

How do I access home care package physiotherapy? 

To access a Home Care Package or Commonwealth Home Supported Package, you will need to have an assessment from my Aged Care. Our physiotherapists will work to develop a plan that includes outlining the type of physiotherapy, duration, frequency and cost to help you work towards improving independence.  

How much does home care package physiotherapy cost?

The cost of Home Care Package physiotherapy will depend on your specific needs and providers. Most physiotherapy services can be fully covered by your Home Care package.

How often will I have home care package physiotherapy appointments?

The frequency of your physiotherapy appointments will depend on your specific needs and treatment plan. Your physiotherapist will be able to outline and discuss this following your initial assessment.

Last week we were fortunate enough to head back to uni and refresh our anatomy knowledge at @latrobeuni in the anatomy labs. 

Thank you to the Latrobe School of Physiotherapy for inviting us! Last week our team celebrated another great year. 

Whilst Melbourne’s weather had its own plans, we were still determined to keep with the theme of ‘Italian Summer’. We let our hair down to master the art of mixology 🍸🍹followed by an Italian feast. Friday morning Run Club with the team. Practicing what we preach!

Staying stronger, healthier and happier. This time last week, our team was nervously completing their final preparations for @melbmara . 

Congratulations to our physiotherapists and ME community who ran on the day.

Dilen and Michael completed their first marathons. Abbey and Lizzie smashed out their first half. Last Sunday, our team headed down to see our own Matt Warren win a premiership in the VAFA Premier Men’s Competition. Last week our team dedicated a full day to professional development and team building. At ME Physio we pride ourselves on collaboration to put you at the centre of everything we do.

Tom is on the return from an ankle dislocation whilst Jude is putting in the hard work following a knee reconstruction. Last week our team enjoyed some friendly rivalry at the footy.
