At ME Physio, we offer physiotherapy, hydrotherapy and a range of exercise services under the Commonwealth Home Support Program (CHSP) and Home Care Packages (HCP). These services are available to people who have a CHSP or HCP and need physiotherapy to assist and help manage their condition to assist in continuing to live and home and independently.  

CHSP and HCP physiotherapy can benefit people to help manage their condition and live independently and actively. This may include:

  • Conditions such as arthritis, osteoporosis
  • Chronic conditions such as heart disease and cancer
  • Neurological conditions including stroke and Parksonson’s disease
  • Alzheimer and Dementia
  • Falls assessment and balance re training.
  • Acute conditions such as back pain or joint pain.

Physiotherapy under the CHSP and HCP can be for ongoing conditions or short-term management of acute conditions. Following you initial assessment, your physiotherapist will develop an individualised plan based on your needs and goals to maintain your independence

CHSP and HCP Funding for physiotherapy  

At ME Physio we work closely and accommodate a range of providers and case managers under CHSP and HCP. Our team will liaise directly with you and your case manager to provide services to assist in maintaining your independence and activity.

After you make your first appointment, our team will send out more information and details regarding your appointment and how to best prepare for your initial consultation.  

Following your initial appointment, our physiotherapists will develop an individualised treatment plan outlining the type of physiotherapy required, duration and frequency of recommended treatment and costings to maintain your independence.

What is Home Care Package physiotherapy? 

Home Care Package physiotherapy provides physiotherapy assessment and treatment that can be fully funded or subsidised by your Home Care Package or Commonwealth Home Support Package.

These services can include a range of physiotherapy treatments and services at ME Physio including physiotherapy, hydrotherapy, pilates and other exercise programs.

Who is eligible for Home Care Package physiotherapy? 

If you have a Commonwealth Home Supported Package (CHSP) or Home Care Package (HCP), you are eligible for physiotherapy treatment to manage or treat conditions that will assist you maintain your independence.

This can include the management and treatment of conditions including:

  • Chronic conditions such as stroke, heart disease or cancer.
  • Musculoskeletal conditions such as arthritis and osteoporosis.
  • Mobility or balance retraining.
  • Acute injuries including back pain, sprains or strains.
How do I access home care package physiotherapy? 

To access a Home Care Package or Commonwealth Home Supported Package, you will need to contact an Aged Care Assessment. NDIS will cover the costs of physiotherapy with our team at ME Physio. Whether you are Self managed or Plan Managed, our physiotherapists will work to develop a plan that includes outlining the type of physiotherapy, duration, frequency and cost to help you work towards improving independence.  

How much does home care package physiotherapy cost?

The cost of Home Care Package physiotherapy will depend on your specific needs and providers. Most physiotherapy services can be fully covered by your Home Care package.

How often will I have home care package physiotherapy appointments?

The frequency of your physiotherapy appointments will depend on your specific needs and treatment plan. Your physiotherapist will be able to outline and discuss this following your initial assessment.

Back to school 🔙📚

Last week we were fortunate enough to head back to uni and refresh our anatomy knowledge at @latrobeuni in the anatomy labs. 

Thank you to the Latrobe School of Physiotherapy for inviting us! Last week our team celebrated another great year. 

Whilst Melbourne’s weather had its own plans, we were still determined to keep with the theme of ‘Italian Summer’. We let our hair down to master the art of mixology 🍸🍹followed by an Italian feast. Friday morning Run Club with the team. Practicing what we preach!

Staying stronger, healthier and happier. This time last week, our team was nervously completing their final preparations for @melbmara . 

Congratulations to our physiotherapists and ME community who ran on the day.

Dilen and Michael completed their first marathons. Abbey and Lizzie smashed out their first half. Last Sunday, our team headed down to see our own Matt Warren win a premiership in the VAFA Premier Men’s Competition. Last week our team dedicated a full day to professional development and team building. At ME Physio we pride ourselves on collaboration to put you at the centre of everything we do. 

Our team covered the latest treatments for back pain, financial well-being and finished off the day with salsa dancing.

For Tony and Joan, healthier and stronger is being able to navigate the iconic Coast to Coast Walk in the UK! We recently had the pleasure of hosting Elise Bujor from Women’s & Men’s Health Physiotherapy to discuss women’s health issues across the lifespan. 

Our team pride themselves on staying up to date to ensure you’re able to stay healthier, happier and stronger. Huge milestones over the past few weeks with both Tom and Jude from @delasallefc doing their first bit of running in their rehab. 

Tom is on the return from an ankle dislocation whilst Jude is putting in the hard work following a knee reconstruction. Last week our team enjoyed some friendly rivalry at the footy. 

With finals around the corner, some of our team can now safely make holidays plans for September. We recently had the pleasure of hosting the podiatry team from @sespodiatry. 

Our physiotherapists Michael and Abbey presented on rehabilitation following Achilles surgical repair and ankle surgery. Last weekend some of our team got together to complete the 10 km run @runmelbourne. There was no finish line picture as some were keen to run another 15 km!

Whilst running wasn’t for some - brunch was well received by everyone.
